Cloudy corneas in adults: a masquerade of phacolytic glaucoma

Abstract Details
Purpose:
To report four cases of phacolytic glaucomas who presented as total cloudy corneas discuss the evaluation, diagnosis, management and prevention of such sight threatening masquerade of complications of cataract
Setting:
Rotary eye Hospital vuyyuru Andhra pradesh India
Methods:
Four patients (males-1 females-3) presented with total loss of vision, complete whiteness of black portion of eye with in one week to 10 days duration, None gave history of trauma, all patients underwent cataract operation in other eye ranging from 5 to 15 yrs ago. Visual acuity in all patients was just perception of light. Lids were odematous, conjunctiva showed ciliary congestion, cornea was opaque and completely cloudy and further details were not visible. IOP was very high and not recordable with applanation tonometer eye ball was stony hard. Ultrasound B scan was normal in all patients. All Patients were diagnosed as having Phacolytic glaucoma, After required medical treatment to control the IOP and anterior chamber inflammation all patients underwent sutureless cataract extraction and IOL implantation.
Results:
Six weeks postoperative best corrected visual acuity in Case1- 6/9, N9, case2 6/12, N9, Case3 6/9, N9 and in Case4 is 6/24, N18
Conclusions:
These cases highlight the sight threatening complications of abnormal delay in cataract surgery and must be kept in mind in differential diagnosis while examining elderly patients who present with cloudy corneas. The authors also discuss the various factors responsible for under utilization of national programmes for control of blindness and suggest the measures to be taken to prevent such untoward incidents.
