Retinal changes on optical coherence tomography in patients on long-term chloroquine or hydroxy chloroquine therapy

Abstract Details
Purpose:
To quantitate the macular and peripapillary retinal nerve fiber layer thickness in patients on long term chloroquine / hydroxy chloroquine therapy using time domain optical coherence tomography (OCT)
Setting:
JIPMER, Puducherry, INDIA (Tertiary hospital)
Methods:
Fifty patients with immunological diseases on chloroquine or hydroxychloroquine for at least 5 years were chosen as cases. Fifty age and sex matched controls were recruited. All patients underwent the following tests: Best corrected visual acuity, dilated fundus examination, red Amsler grid test, Humphrey's 10-2 standard achromatic perimetry, macular thickness and peripapillary retinal nerve fiber layer thickness using Fast protocols on Stratus OCT.
Results:
The macular thickness in the parafoveal region was lesser in patients on antimalarials compared to the controls especially in the superior and inferior quadrants which reached statistically significant levels. (Superior: 252 ±34 vs. 261 ± 23, p = 0.033 and inferior: 252 ±32 vs. 262 ± 26, p = 0.016). Other screening tests of retinopathy were normal. There was no significant RNFL thinning in peripapillary region of study patients. There was no correlation between the dose of the drug and thickness of retina. Conclusions:
OCT should be a part of the routine screening for chloroquine retinopathy FINANCIAL INTEREST: NONE
