Comparison of blood pressure change between traditional phacoemulsification surgery and femtosecond laser-assisted cataract surgery

Abstract Details
Purpose:
To evaluate blood pressure changes of pre-operative and post-operative cataract surgeries, done by the same surgeon via two different surgical methods.
Setting:
Universal eye center, Taipei, Taiwan
Methods:
Retrospective chart review was performed for all patients received traditional phacoemulsification (TP) surgery (Alcon Infiniti System) and femtosecond laser-assisted (FLA) cataract surgery (Alcon-LenSx, Aliso Viejo, CA; combined with Alcon Infiniti System) in 2013. The same surgeon completed all the surgeries smoothly by implantation different intraocular lens (IOL). Patient characteristics (age, gender, hypertension history), pre-operative blood pressure (BP), and post-operative blood pressure were collected.
Results:
Sixty-seven patients involving of one hundred and three eyes receiving two types of operations were included. Average age of patients was 63.6 years old; Twenty-nine patients in this study had hypertension history and twenty-six cases received regular medications control. Thirty-nine eyes received TP surgery and the other sixty-four eyes received FLA cataract surgery. Average surgical time was 14.77 minutes in TP surgery group and 25.47 minutes in FLA surgery group. In TP surgery group, average pre-operative systolic BP and diastolic BP were 125.41mmHg and 71.49 mmHg; post-operative systolic BP and diastolic BP were 130.28 mmHg and 73.05 mmHg. In another group, average pre-operative systolic BP and diastolic BP were 127.17 mmHg and 73.56 mmHg; post-operative systolic BP and diastolic BP were 134.17 mmHg and 78.11 mmHg respectively. Paired-samples T test shows obvious BP elevation after FLA cataract surgery (P value=0.01) and no reliability in TP group (P value= 0.094). Generalized linear model reveals BP elevation which being related to longer surgical time in FLA cataract surgery group (P value= 0.009).
Conclusions:
This study revealed of blood pressure increased after femtosecond laser-assisted cataract surgery. Femtosecond laser-assisted cataract surgery is a high-priced premium new type cataract surgery and it is supposed to be a more precise, more accurate, more reproducible, more predictable and safer surgery course than traditional phacoemulsification surgery. Usually, it needs more surgical time than traditional phacoemulsification. Patients maybe accept more physical stress for the cause of longer surgical time. Hemodynamic change during surgery should be considered for some high-risk patients and monitoring should be implemented during surgery for every patients. FINANCIAL INTEREST: NONE
