Higher-order aberrations after femtosecond LASIK: wavefront-guided vs non wavefront-guided ablations

Abstract Details
Purpose:
To analyze and compare changes of higher-order aberrations after standard and wavefront-guided LASIK at 1, 3, and 12 months postoperatively.
Setting:
European Eye Clinic Lexum, Brno, Czech Republic
Methods:
Study of 200 consecutive eyes of myopic patients (-0.5 to -8.0D), who underwent standard non-wavefront-guided LASIK (nonWG LASIK) (100 eyes) or wavefront-guided LASIK (WG LASIK) (100 eyes) using femtosecond laser LDV Ziemer and excimer laser MEL 80 Zeiss with iris registration. Primary outcome measures were manifest refraction spherical equivalent (MRSE), uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA) and higher-order ocular aberrations (HOAs). HOAs measured with Hartmann-Shack wavefront aberrometer WASCA, HOAs analysed at 6 mm pupil, assessed total HOA root mean square (RMS) and individual Zernike coefficients.
Results:
Preoperatively, there were no significant differences between the nonWG and WG groups in MRSE, UDVA, CDVA or HOAs. Postoperatively, WG LASIK achieved lower RMS HOAs at 1 month (p=0.003) and 3 month (p=0.005) and better CDVA at 3 month (p=0.045). Significant differences were not found between treatment groups in MRSE, UDVA, CDVA or HOAs at 1 year.
Conclusions:
Both wavefront-guided and non-wavefront-guided LASIK with LDV and MEL 80 platform have shown very good efficacy and safety. Myopic LASIK only slightly increase RMS HOAs, especially by induction of spherical aberration. Both methods have equivalent postoperative aberration score one year postoperatively. However, wavefront-guided LASIK had better postoperative aberration profile at 1 and 3 month postoperatively. FINANCIAL INTEREST: NONE
