Long-term laser blended vision results for myopic, hyperopic and emmetropic presbyopia

Purpose:

To evaluate the long term monocular and binocular visual outcomes of Laser Blended Vision®LASIK protocol Laser Blended Vision is an aspheric micro-mono-vision presbyopia correction protocol.

Methods:

A retrospective, no comparative case series included 558 eyes of 279 consecutive patients with myopic presbyopia (Group 1), 235 eyes of 117 consecutive patients with hyperopic presbyopia (Group 2) and 161 eyes of 81 consecutive emmetropic presbyopia (Group 3) who were treated with LASIK-induced micro-monovision. The Carl Zeiss Meditec CRS-Master software and MEL 80 excimer laser were used. The target refraction was plano for distance eyes (dominant eye) and between -0.75 and -1.50 diopters (D) for near eyes. 16 % of patients in group 1 were followed for 1 year and more, 20 % in group 2 and 22% in group 3. Mean attempted spherical equivalent refraction (SE) correction for group 1, 2 and 3 was respectively -4.08 ± 2.30 D (range: -0.25 to -11.125 D), +1.76 ± 1.02 D (range: +0.75 to +6.25 D) and +0.33 ± 0.34 D (range: -0.25 to +0.875 D). Mean attempted cylinder correction for group 1, 2 and 3 was respectively 0.76 ± 0.70 D (range: 0 to 3.50 D), 0.43 ± 0.58 D (range: 0.25 to 4.50 D) and 0.41 ± 0.36 D (range: 0.25 to 1.25 D).

Results:

Outcome measures after 1 year were as follows. SE regression was 0.75D or higher in 2% of group 1 (1 eye), 20% in group 2 and 17% in group 3. The regression of group 2 and 3 was accompanied by a significant keratometry variation. The average change in keratometry was 0.51D for group 2 and 0.38D for group 3. Binocular uncorrected distance visual acuity for group 1, 2 and 3 was respectively 20/20 in 82% and 20/25 in 95%, 20/20 in 78% and 20/25 in 100% and 20/20 in 100%. Binocular uncorrected near visual acuity for group 1, 2 and 3 was respectively 20/20 (P1) in 60% and 20/25 (P1.5) in 100%, 20/25 (P1.5) in 72% and 20/32 (P2) in 88% and 20/25 (P1.5) in 66% and 20/32 (P2) in 83%. Binocular uncorrected visual acuity for group 1, 2 and 3, was respectively 20/25 and P1.5 in 95%, 20/25 and P2 in 76% and 20/20 and P2 in 83%. . No eyes lost 2 or more line of corrected distance visual acuity in group 1 and 3, one patient lost 2 lines of corrected visual acuity in group 3.

Conclusions:

Laser Blended Vision protocol is stable for myopic presbyopia treatment. A small part of regression is observed in emmetropic and hyperopic presbyopia treatments. It is effective with good results in binocular UDVA and UNVA. And it is well tolerated with a good security.
