Six month results of a clinical evaluation of pseudophakic presbyopes treated with a presbyopic LASIK algorithm

Purpose:

A prospective clinical study to evaluate the safety and efficacy of the Supracor presbyopic LASIK algorithm in pseudophakic patients implanted with monofocal intraocular lenses.

Setting:

Asian Eye Institute, Philippines

Methods:

In this single centre, prospective study, 30 pseudophakic patients implanted with monofocal IOLs underwent a unilateral LASIK procedure for the treatment of presbyopia (Supracor, Bausch + Lomb Technolas). Following 120µm microkeratome flap creation, all Supracor procedures were performed with the Technolas 217P Excimer laser. All patients were 45 years and over, had a refractive error between -2.0D and +2.0D, cylinder up to 2.0D and best corrected distance visual acuity of at least 20/25. Safety, efficacy, uncorrected distance, intermediate and near visual acuity (UDVA, UIVA, UNVA), best correct distance visual acuity (BSCVA), patient questionnaire and retreatment rates over a 6 month follow-up period were assessed. Target refraction was -0.5D.

Results:

Mean patient age was 68.7 years. Pre-operative mean refractive spherical equivalent (MRSE) was 0.28D and cylinder was -0.85D. At 6 months follow-up, MRSE was -0.54D and cylinder was -0.71D. Monocular UDVA remained stable at 6 months post-op with 66% at 20/25 compared to 63% at 20/25 pre-operatively. Binocular UDVA improved from 77% at 20/20 pre-operatively to 97% at 6 months post-op. Monocular and binocular UIVA improved with 59% at 20/20 binocular UIVA at 6 months post-op versus 33% pre-operatively. Likewise, improvements in UNVA were observed, with 48% at binocular UNVA of 20/20 compared with 7% pre-operatively. Patients lost an average of -0.22 lines of monocular CDVA and 1 patient received YAG treatment during the follow-up period. 96.6% patients report being able to read the menu and 93.1% can read text messages without glasses. 79.3% report improved night vision, 17.2% report it as being the same and 3.4% consider it to be worse at 6 months post-op. Only one patient required a retreatment procedure, which was successfully performed.

Conclusions:

The 6 months data indicates the Supracor LASIK algorithm is safe and effective in pseudophakic patients implanted with a monofocal IOL. Uncorrected near and intermediate vision are improved, whilst maintaining or improving uncorrected distance vision. The procedure also allows the possibility for a retreatment.
