LASIK for the correction of high myopia from -8.00 to -14.00 D in 481 eyes with a minimum of 1 year follow-up

Abstract Details



Purpose:

Report the visual outcomes of high myopic LASIK using the MEL80 excimer laser.

Setting:

London Vision Clinic, London, UK

Methods:

Retrospective analysis of 481 consecutive high myopic LASIK procedures (319 patients) using the MEL80 excimer laser non-linear aspheric profile and either the Hansatome microkeratome or VisuMax femtosecond laser. Inclusion criteria were attempted maximum myopic meridian ≥-8.00D, CDVA 20/20 or better, and minimum follow-up of 1 year. Standard outcomes analysis, including contrast sensitivity, was performed for the primary procedure and including retreatments. The primary procedure was a deliberate partial correction in 79 eyes (16%), of which 56 (71%) have undergone a retreatment to date. Of the 402 eyes intended for full correction, 64 (16%) have undergone a retreatment. Prior to retreatment, residual stromal bed thickness minimum was determined by Artemis VHF digital ultrasound. A retreatment was performed if the predicted residual stromal thickness was >250 µm at the location of the maximum ablation.

Results:

Mean attempted SEQ was -9.38±1.22D (-7.84 to -14.18D) for the primary procedure, and -9.57±1.29D (-7.50 to -14.18D) including all procedures. Preoperatively, mean refractive astigmatism was 1.03±0.84D (0.00 to 4.50D), mean age was 36 years (19 to 60 years), mean average keratometry was 43.9±1.6D (39.4 to 48.5D). Pupil size min-max, median?. After all treatments, SEQ relative to the intended target was -0.26±0.47D (-2.25 to +1.13D) with 69% ±0.50D, 95% ±1.00D. Mean SEQ was -0.29D at 3 months, -0.37D at 6 months, -0.29D at 1 year and -0.39D at 2 years. UDVA was 20/20 or better in 89% and 20/40 or better in 99% of eyes. There was a loss of 1 line of CDVA in 3%, and no eyes lost ≥2 lines. There was no decrease in mesopic contrast sensitivity at 3 and 6 cpd and a small but statistically significant increase in mesopic contrast sensitivity at 12 and 18 cpd (p<0.05). Postoperatively, mean average keratometry was 38.3±1.5D (32.9 to 42.8D). A topography-guided retreatment was performed due to the patient complaining of night vision disturbances in 8 eyes (1.7%) and these patients were all satisfied with their night vision after this procedure.

Conclusions:

LASIK ablation for myopia up to -14.00 D with the MEL80 excimer laser was found to be safe, effective and stable.
