Efficacy of anti-vaulting haptic design for rotational stability of toric lens over two years: prospective photo-analysis study of 12 eyes

Purpose:

To determine the rotational stability of hydrophilic Rayner toric IOL with Anti-vaulting haptic design by comparing retro-illumination photographs taken at 1 week, 6 and 24 months post operatively. To determine the change in rotation between 6 and 24 months.

Setting:

Scarborough hospital, York Teaching hospital, UK

Methods:

In a prospective study of 12 eyes undergoing phacoemulsification surgery, Rayner T-flex toric lens was implanted in the bag and its axis markings were aligned with the axis marked at the limbus. T-flex IOL has anti-vaulting haptics designed to resist the capsule contraction forces. Retro-illumination photographs were taken on slit lamp at 1 week, 6 and 24 months. The photographs were analysed on Adobe Acrobat software by drawing horizontal axis and axis across the markings. The angle between the two axes was determined by the software up to two decimal values. The change in angle from 1 week to 6 and 24 months was calculated along with mean rotation at 6 and 24 months. This was grouped in to <5 degree, 5-10 degree and > 10 degree.

Results:

The range of torus corrected was +1.5D to +6.5D. At 24 months, none of the 12 eyes showed rotation of more than 5 degrees on retro-illumination photographs. The maximum rotation noted was 4.52 degrees. Between 6 and 24 months, 3 eyes showed negative rotation while 9 eyes showed further rotation. Mean rotation at 6 and 24 months was 2.32 and 3.17 degrees respectively. At 24 months, 10 eyes achieved 20/40 or better unaided vision. 11 patients were highly satisfied with their unaided vision while 1 patient was neutral.

Conclusions:

Rayner T-flex toric lens with anti-vaulting haptics showed excellent rotational stability at 6 and 24 months following surgery. Rotation between 6 and 24 months was not significant. Excellent unaided visual acuity and high satisfaction rate were noted in these patients.
