Customizing near vision ADD with an extended range of Tecnis multifocal IOLs

Abstract Details
Purpose:
An extended range of Tecnis multifocal IOLs with near vision adds of 2.75 (ZKB00), 3.25 (ZLB00) and 4 (ZMB00) dioptres has recently been introduced. The light distribution remains the same but this range of near vision add translates to a range of near vision add in the spectacle plane . This effective spectacle plane add for a given IOL add depends on the Keratometry and AC depth of an eye. We present our results of using a strategy of choosing a Tecnis Multifocal IOL based on the IOL add power needed to produce a spectacle plane add power of 2.5 D for a given eye.

Methods:
20 Eyes undergoing multifocal IOL implantation for cataract or refractive lens exchange had a Tecnis multifocal IOL implanted based on the ideal IOL add power needed to produce a spectacle plane add of 2.5 D. This calculation was done using the Holladay Consultant software. Thus if a 4D IOL add was needed to produce a 2.5 d spectacle plane add the Tecnis ZMB00 which has an add power of 4D was chosen while if a 3.25 D IOL add power was needed for an individual eye the Tecnis ZLB00 which has an add power of 3.25 D was chosen. If the required add power was in between the Tecnis model add powers the nearest IOL add power was chosen.
6-8 weeks post op all eyes had a full PANFOCAL vision assessment which included their subjective refraction, uncorrected and distance corrected photopic distance, intermediate and near visual acuity, mesopic near visual acuity and an objective task assessment of intermediate and near vision tasks in photopic and mesopic conditions. A Subjective patient questionnaire was also used to determine patient perception of their vision at different distances.
Results:
The Tecnis ZLB00 was the commonest model chosen most eyes needed an IOL add power in the range of 3.25 to 3.65 D. 3 eyes needed a Tecnis ZMB00 model. There were no differences in the reading distances and there were no significant differences in the PANFOCAL vision assessment between the two models however the Tecnis ZMB00 lens showed a trend towards better mesopic near vision and worse intermediate vision. Subjective patient questionnaire showed very satisfaction with the near vision performance.
Conclusions:
The extended range of Tecnis Multifocal IOLs provides a good toolkit for surgeons to optimise the near vision add in individual eyes.
