Functional outcomes for the Lenstec SBL-3 Dual Optic rotationally asymmetric refractive multifocal intraocular lens

Abstract Details



Purpose:

To evaluate visual performance for far, intermediate and near vision after implantation of a new asymmetrically refractive multifocal intraocular lens (MIOL) with a +3.50 D near addition power.

Setting:

Cathedral Eye Clinic (Private practice), Belfast, Northern Ireland, UK.

Methods:

A total of 23 cataract patients were bilaterally implanted with the SBL3 MIOL in one centre by one experienced surgeon (JM) and followed for 3 months. Main outcome measures included monocular uncorrected and corrected distance visual acuities (UCVA and BCVA) with logMAR ETDRS Chart 1 original at 4m, uncorrected and best distance corrected intermediate (UCIVA and BDCIVA) (70cm) and uncorrected and best distance corrected near (UCNVA and BDCNVA) visual acuities (Radner reading charts at 40cm) under a standard mesopic lighting condition (85cd/m2). Radner charts allow direct conversion (0.2 logMAR distance acuity is comparable to 0.2 logRAD reading acuity) with high correlation at 40 cm to a logMAR equivalent for size of letters. Secondary outcome measures included monocular defocus curves (technique previously described by Alio et al) using Radner reading charts to simulate distance visual acuity at 33cm, contrast sensitivity (CS) measured with Pelli-Robson Contrast Sensitivity Chart at 1m and 85cd/m2and patient reported outcomes collected with Quality of Vision (QoV) and Quality of Life (QoL) validated questionnaires.

Results:

Three months postoperatively, 46 eyes of 23 patients (mean age 65 ± 4.9 years) achieved monocular uncorrected distance visual acuity (mean ± SD; logMAR) of 0.14 ± 0.13, uncorrected intermediate visual acuity of 0.19 ± 0.11 and uncorrected near visual acuity of 0.24 ± 0.14. With best distance correction, patients achieved -0.05 ± 0.08 for distance, 0.10 ± 0.14 for intermediate (70cm), and 0.12 ± 0.15 for near (40 cm). There were no surgical complications. Monocular and binocular defocus curves showed 2 peaks of maximum VA at the distance focus (0.0 D) and the near focus (−2.5 D) and a good range of intermediate vision with the least mean acuity (0.18± 0.08 logMAR) found at −1.5 D defocus. Mesopic CS were within the age acceptable limits. The majority of patients (n = 20; 86.9%) were glasses free; 8.7% (n = 2) reported wearing glasses occasionally for very small print. Overall, all patients were 'very satisfied' (n = 21; 91.3%) or 'satisfied' (n = 2; 8.7%) with the procedure. Isolated night vision related problems were not encountered.

Conclusions:

The new SBL-3 MIOL safely provides a predictable range of near to far vision. The majority of patients are highly satisfied and spectacle independent at all distances. FINANCIAL INTEREST: NONE
