Outcomes of a toric trifocal diffractive intraocular lens

Purpose:
Preliminary prospective evaluation of refractive and visual outcomes following implantation of a Toric Trifocal Diffractive Lens Implant, the AT Lisa Trifocal Toric following cataract surgery or refractive lens exchange
Setting:
Centre for Sight, Hazelden Place, East Grinstead, UK - Private ambulatory surgical centre,
Methods:
The data of 10 consecutive patients (20 eyes) undergoing bilateral implantation of the AT Lisa Trifocal Toric lens implant (Zeiss, Jena, Germany) following femtosecond laser (Victus laser, Bausch and Lomb-Technolas, Munich Germany) will be analysed. Visual data (UDVA, CDVA, UNVA and Uncorrected intermediate vision at 60 and 80cm) and refractive outcomes will be collected and contrast sensitivity testing will be performed at 3 months follow-up.
Results:
Analysis of initial 1 month data on 8 eyes of 4 patients showed a mean age of patients undergoing surgery at 59 years (51 to 67). No surgical complications were reported. All toric lenses were stable and within 5 degrees of intended axis of implantation. One patient developed capsular block syndrome at 1 month requiring an anterior Yag capsulotomy. Preoperatively the mean astigmatism was -2.25 D ± 1.31 D -1.00 to -6.00) and mean spherical equivalent -3.17 D ± 5.03 D (-8.13 to +7.13). The mean postoperative spherical equivalent reduced to -0.05D ± 0.15(-0.25 to 0.13) and mean postoperative astigmatism was -0.28 D ± 0.21 D (-0.50 to 0.00). 100% of eyes had monocular UDVA of 6/7.5 or better and 75% were 6/6 at 1 month. Monocular UNVA at 40cm was 6/9 or better in 100%, 6/7.5 or better in 75% and 6/6 in 63%. UIVA at 60cm was 6/9 or better in 100%, 6/7.5 or better in 88% and 6/6 in 25%. UIVA at 80cm was 6/7.5 or better in 100%, and 6/6 or better in 50%. All patients reported to be free of spectacles and able to drive at night without significant symptoms of halos or glare.
Conclusions:
The AT Lisa Diffractive Trifocal Toric lens implant is extremely stable once implanted and provides excellent refractive outcomes with highly predictable and reliable reduction in cylinder as well as good visual outcomes at distance, intermediate and near.
