The role of ultrasound biomicroscopy in eyes with silicone oil endotamponade

Purpose:

The purpose of the study was to evaluate the utility of ultrasound biomicroscopy (UBM) in the assessment of anterior segment structures in vitrectomised eyes with silicone oil endotamponade.

Setting:

University department of Ophthalmology, Clinical hospital center 'Sestre milosrdnice', Zagreb, Croatia

Methods:

Thirty-eight patients treated with pars plana vitrectomy and silicone oil were enrolled in the study. Both, the study eye and the contralateral healthy eye, were subjected to the same examination regimen. The contralateral healthy eye was used as control. Before the UBM examination, slit-lamp biomicroscopy, gonioscopy, intraocular pressure measurement and dilated fundus examination was performed. The UBM examination was performed in supine position using 50-MHz probe. Each eye was studied as follows: 1. central scan in vertical (sagittal) axis; 2. Central scan in the horizontal (axial) axis; 3. four radial scans centered at limbus in the following sequences of hour meridians-first, 9 hours, second, 12 hours, third, 3 hours, fourth, 6 hours; 4. Corresponding radial scans centered at periphery; 5. Freezing and storing of images; 6. Image analysis.

Results:

The mean age of the patients was 63.7 + 9.5 years (range 27-84 years). In 11 of the 38 eyes gonioscopy did not detected anterior chamber oil emulsification. UBM showed increased reflectivity of angular structures in all vitrectomised eyes, especially in the superior part of anterior angle. The small droplets on the angle structures, trapped in the trabecular meshwork with typical hiperreflectivity, were detected in 36 from 38 eyes. Uveoscleral space showed presence of tiny, highly reflective particles in 27 of 38 eyes. In 32 from 38 eyes we detected highly reflective smaller and bigger droplets floating in the anterior chamber. The anterior and posterior surface of the iris was impregnated with highly reflective pattern in all studied eyes. A typical hyperreflectivity was also found in the posterior chamber, ciliary body, periphery and in the phakic eyes, lens and zonule. None of the control eyes showed any pattern.

Conclusions:

Ultrasound biomicroscopy (UBM) is an objective and reliable diagnostic method, which can be used to observe the changes of anterior segment structures in silicone filled eyes. These findings may help in management as well as a better understanding of the silicone oil effect on the anterior segment structures in the vitrectomised eyes with silicone oil endotamponade.
