Correlation of anterior and posterior corneal elevation and asphericity with severity of keratoconus

Purpose:

To investigate the severity of keratoconus in terms of anterior and posterior corneal elevation and asphericity

Setting:

Bir Inci Eye Hospital, Istanbul, Turkey

Methods:

Three-hundred and twenty seven keratoconus eyes with mean age of 31.1 9.7 years were evaluated. Anterior and posterior corneal surface elevations of the thinnest point and anterior-posterior corneal asphericity of 8 mm central cornea were measured using Scheimpflug tomographic analysis. The data correlated with keratoconus severity index (Pentacam®-derived Amsler - Krumeich stages).

Results:

Anterior best fit sphere showed significant positive correlations with keratoconus severity whereas, anterior corneal assphericity showed significant negative correlations with keratoconus severity ( Pearson correlation coefficient, r=0.686; P<0.001 and r=-0.596:P<0.001). Posterior best fit sphere showed low correlation with keratoconus severity ( r=0.303; P<0.001). There were no significant correlation between posterior asphericity and keratoconus severity.

Conclusions:

Anterior corneal surface height and asphericity had the strongest correlation with topographic keratoconus grading and should be considered an additional factor in keratoconus grading. FINANCIAL INTEREST: NONE
