Corneal morphology evaluation by OCT and confocal microscopy after phacoemulsification surgery

Purpose:

The purpose of the study was the corneal morphology analysis using time domain optical coherence tomography (TD OCT) and confocal microscopy before and after 2,75 mm phacoemulsification surgery.

Setting:

II Ophthalmology Clinic Silesian Medical University; Katowice: Poland

Methods:

120 patients undergoing routine phacoemulsification surgery were included in the study. Inclusion criteria were: cataract stage I-IV, no accompanying ophthalmic diseases and no systemic diseases which presence or treatment affect cornea. TD OCT and confocal microscopy examination was performed preoperatively and on the first and seventh day postoperatively. The central and peripheral corneal thickness, integrity and the morphology of the corneal layers was assessed. The endothelial cell density was calculated.

Results:

OCT analysis revealed a transient peripheral corneal edema in 95% of analyzed corneas, local Descemet membrane detachment in 4,16 % and would leakage in 0,83 % of patients. The most common confocal microscopy finding was cornea guttata revealed for 13,75% of patients. Other less frequent corneal features included: anterior corneal microdots, mall dendritic cells and striae in the posterior corneal part. After phacoemulsification confocal microscopy revealed changes as follows: micro erosion, stromal edema and local Descemet membrane detachment on the first postoperative day. The endothelial cell lost of 6,5% was revealed.

Conclusions:

The corneal morphology changes after phacoemulsification surgery not visible on the slit lamp examination could be monitored with optical coherence tomography confocal microscopy FINANCIAL INTEREST: NONE
