Pupillometry in a study population with healthy and with glaucomatous eyes under different lighting conditions

Purpose:

The aim of the study was to analyze pupil sizes under different lighting conditions (scotopic, mesopic and photopic) for a young and healthy normal population to derive normal values as well as for a population with manifest clinical glaucoma, and to calculate a measure for reliability for the instrument for measuring normals.

Setting:

Consecutive cross-sectional monocentric case series performed at Department of Ophthalmology, University Medical Center Homburg/Saar, Germany.

Methods:

Pupil size was measured in 102 young healthy volunteers (normals) and 114 glaucoma patients bilaterally with a commercially available pupillometer (PupilX, Albomed, Germany). We restricted to right eyes only. Each measurement sequence included monitoring of a time interval of 30 seconds with a frame rate of 30 Hz for 3 different lighting conditions (scotopic, mesopic and photopic). Within this time period pupil size data were averaged and standard deviation (STD) and median (MED) were documented. For the normals a set of 5 complete measurement sequences was taken to derive measures for reliability of the device.

Results:

Mean pupil size data in normal were 5.81±0.91 mm (MED/STD: 5.83/0.06 mm) / 4.89±0.73 mm (MED/STD: 4.97/0.13 mm) / 3.59±0.73 mm (MED/STD: 3.53/0.17 mm) for scotopic/mesopic/photopic conditions, respectively. For the glaucoma eyes respective parameters were 4.61±1.10 mm (MED/STD: 5.29/0.13 mm) / 4.12±1.01 mm (MED/STD: 3.99/0.16 mm) / 3.41±0.91 mm (MED/STD: 3.27/0.17 mm). Cronbach's alpha as a measure for reliability for the normal were 0.995/0.989/0.980 for scotopic/mesopic/photopic conditions. Pupil size in normal was significantly larger compared to glaucoma eyes (p<0.001, p<0.001/p=0.008).

Conclusions:

The presented pupil size data could be used as normal values for further studies with the PupilX. As Pupil size in glaucoma eyes are significantly smaller compared to the normal population, pupil measurements could be used as supplementary diagnostic tool for documentation of clinical glaucoma. Fluctuation during the measurement interval is very low, which implicates that the interval could be shortened for screening applications. The reliability of the instrument seems to be extremely high. FINANCIAL INTEREST: NONE
