Scleral granuloma revealing a foreign body at the ciliary body

Purpose:

Against any ocular trauma, the presence of an intraocular foreign body (foreign body) should be suspected even in the absence of obvious because wound infection risk and metallosis Some locations of foreign bodies are difficult to diagnose, especially those of the anterior chamber angle and ciliary body.

Setting:

Hopital des Specialités in Rabat City- Morocco

Methods:

We report about this, atypical case of a child with scleral granuloma and in which high-frequency ultrasound (UBM) revealed the existence of a plant foreign body at the ciliary body. This is a 6 year old child who consulted the emergency for a red eye appearance (OD). In its history, there is the notion of an accidental injury by a plant thorn in the right eye, neglected, a month ago. Treatment with anti inflammatory drugs and topical antibiotics was introduced before admission in our training, but without improvement. The ophthalmological examination at the slit lamp revealed at the right eye a punctate corneal opacity near the limbus with a scleral nodule formation and yellowish round protruding anterior chamber fibrin appearance reminiscent of a granuloma. Against this clinical picture was suspected a foreign body of vegetable origin at the ciliary body or angle iridocornéeen probably a fungal infection.

Results:

The gonioscopic review did not objectified foreign body, The radiograph was normal, Ocular B-mode ultrasound with a probe 10 Hz was also normal. The UBM showed a foreign body at the ciliary body whose shape evoked a plant thorn. The patient was treated with corticosteroids and local antibiotic therapy, surgical exploration was planned to remove the foreign body, however, the evolution was marked regression of granuloma under treatment with fistula and spontaneous expulsion of foreign body.

Conclusions:

This case highlights the diagnostic difficulties of a foreign body ciliary body unnoticed and the contribution of UBM in eye trauma for the diagnosis and localization of the foreign body from anterior segment of the eye. FINANCIAL INTEREST: NONE
