Novel grading of posterior polar cataract with anterior segment optical coherence tomography

Purpose:
Using anterior segment optical coherence tomography (ASOCT) to identify eyes with posterior polar cataract at high risk of posterior capsular rupture (PCR) during cataract extraction
Setting:
Hong Kong Eye Hospital
Methods:
Thirty-seven eyes with clinically diagnosed posterior polar cataract and ASOCT images, which underwent phacoemulsification, were included. Each eye was graded according to features of the posterior lens opacity and its underneath capsule. Eyes were categorized as Grade 1 or 2 depending on the amount of clearance between the posterior opacity and the capsule with 50% or more clearance in Grade 1 and less than 50% in the latter. Grade 3 represented the absence of an intact posterior capsule. Rate of intraoperative PCR was compared with the ASOCT grading in order to identify eyes at high risk of PCR.
Results:
The PCR rate of the current series was 21.6%. There were 19, 13 and 5 eyes of Grade 1, 2 and 3, respectively. The corresponding numbers of PCR were 1 (5.3%), 4 (30.8%) and 3 (60.0%), respectively. The PCR rate was significantly different among groups (p = 0.018). By combing the eyes of Grade 2 and 3, ASOCT had a sensitivity of 87.5% and specificity of 62.1%.
Conclusions:
ASOCT can be used to grade eyes with posterior polar cataract. It is a sensitive screening tool to identify eyes at high risk of PCR and allow for better operation planning as well as pre-operative counseling. FINANCIAL INTEREST: NONE
