Comparison of surgically-induced astigmatism between trabeculectomy and ExPRESS shunt

Abstract Details



Purpose:

There have been few reports on surgically induced astigmatism after trabeculectomy or ExPRESS shunt. We retrospectively examined cases with surgically induced astigmatism after either of these procedures.

Setting:

Department of Ophthalmology, Kurume University Hospital.

Methods:

This study included 81 eyes of 73 patients who underwent trabeculectomy or ExPRESS shunt as an initial glaucoma surgery. Thirty-four eyes of 31 patients underwent trabeculectomy (trabeculectomy group). Forty-seven eyes of 42 patients underwent ExPress shunt (ExPRESS group). Corneal thickness was measured with the Pentacam® (Oculus, Wetzlar, Germany), and preoperative and postoperative corneal astigmatism was measured with the Auto Kerato-Refractometer KR7100® (Topcon, Tokyo, Japan). Surgically induced astigmatism at 1 month and at 6 months was calculated with the SIA Calculator Version 1.1©2010.

Results:

Preoperative intraocular pressure (IOP) was 30.5±8.1 mmHg in trabeculectomy group and 26.6±5.7 mmHg in ExPRESS group. Postoperative IOP in trabeculectomy group and ExPRESS group was 13.1±5.4 mmHg and 10.6±5.9 mmHg at one month and 13.2±3.7 mmHg and 10.8±3.5 mmHg at six months, respectively. Postoperative IOP was significantly decreased compared with preoperative IOP. Surgically induced astigmatism in trabeculectomy group and ExPRESS group was 2.0±1.4 D and 2.1±1.9 D at one month and 1.2±0.7 D and 1.5±1.0 D at six months, respectively, with a trend toward decreasing amount of astigmatism over time. We evaluated factors for age, final visual acuity, IOP, change in IOP (preoperative IOP-postoperative IOP), and corneal thickness. There was no correlation among variables and no significant difference between the groups.

Conclusions:

There was no difference in surgically induced astigmatism between trabeculectomy and ExPRESS shunt, and the astigmatism gradually decreased with time. FINANCIAL INTEREST: NONE
