Features of a condition of eyes with primary open-angle glaucoma before phacoemulsification

Abstract Details

Purpose:

to reveal the features complicating phacoemulsification surgery

Setting:

The Khabarovsk branch of the State Institution Eye Microsurgery Complex named after S.N. Fyodorov, Khabarovsk, Russia

Methods:

61 patients (61 eyes) with combination of primary open-angle glaucoma (POAG) and age-related cataract for whom phacoemulsification was planned. Age - from 56 to 87 years. Men - 29, women - 32. In all patients permanent normalization of intraocular pressure (IOP) was reached. Initial stage of POAG - 17 eyes, developed - 12, hard - 32. Level of IOP made from 10 to 15 mm Hg, on the average - 12,2 mm Hg. In 34 eyes normalization of IOP was reached after antiglaucoma surgeries. In 27 eyes - due to instillations of hypotensive preparations. Any anatomo-morphological factors creating risk of development intra- and postoperative complications after phacoemulsification were revealed.

Results:

In all 12 eyes after antiglaucoma surgeries of penetrating type the narrow, rigid pupil was noted: in 7 eyes there were extensive back pigmentary synechia, the pupil didn't extend after mydriatics instillations. In 5 eyes maximum medicamentous mydriasis made 4-5 mm. The lens incomplete dislocation, defects of Zinn's ligament tissues of ¼-½ circles were found in 4 eyes. In all 12 eyes after antiglaucoma surgeries of penetrating type the dense nucleus of a lens was noted. In 20 eyes after antiglaucoma surgeries of non-penetrating type the rigid pupil was revealed, in 17 eyes - mydriasis to 5 mm, in 14 eyes - a dense nucleus of a lens, in 6 eyes - the lens incomplete dislocation. In 16 eyes on a hypotensive regimen the rigid pupil was noted (maximum mydriasis to 5-5,5 mm), in 13 eyes - a dense nucleus of a lens, in 3 eyes - a lens incomplete dislocation took place.

Conclusions:

1. At combination of POAG and cataract the increased risk of complications of a phacoemulsification takes place in 79%. 2. Most often met: rigid pupil, lens incomplete dislocation; the increased density of a lens nucleus, back pigmentary synechia. FINANCIAL INTEREST: NONE
