Efficiency of ExPRESS (Alcon) device implantation in the complex treatment of open-angle glaucoma combined with cataract, depending on the severity of glaucoma

Abstract Details
Purpose:
The aim of this study is to estimate the efficacy of the EX-PRESS microdraining device implantation in complex treatment of open-angle glaucoma combined with cataract depending on the stage of the glaucoma process.
Setting:
The system of medical centers «Noviy Zir»
Kyiv, Cherkassy. Ukraine
Methods:
73 patients (73 eyes) were divided into two groups depending on the method of the glaucoma surgery. The 1st group included 41 patients (41eyes) who underwent the EX-PRESS microdraining device (diameter 50 µm) implantation combined with phacoemulsification. In the 2nd group, which included 32 patients (32 eyes), non-penetrating sclerectomy combined with phacoemulsification was performed . We performed visometry, tonometry, gonioscopy, ophthalmoscopy, pachymetry and optical coherence tomography (the ganglion cells complex and the nerve fiber layer thickness) before the operation and 24 months after the surgery.
Results:
According to the data, the glaucoma process stabilization was achieved in 97% of cases of mild (stage 1) and moderate (stage2) glaucoma in the group where EX-PRESS mini shunt implantation combined with the phacoemulsification was performed. Stabilization of the glaucoma process was achieved in 92 % of cases in stage 1-2 glaucoma in the group where non-penetrating sclerectomy was performed. In the patients with advanced (stage 3) glaucoma stabilization was achieved in 93% of cases in group 1, and in 89% in the 2nd group.
Conclusions:
Long-term gradual IOP reduction and safety of Ex-PRESS glaucoma device implantation may be the reason of choice of this surgery method both for patients with mild- and moderate-stage glaucoma and with advanced-stage glaucoma. FINANCIAL INTEREST: NONE
