The comparative assessment of influence ExPRESS device implantation versus ExPRESS device implantation combined with phacoemulsification on intraocular pressure in glaucomatous eyes

Abstract Details
Purpose:
To evaluate two surgery methods: Ex Press device implantation and Express device implantation combined with phacoemulsification for open-angle glaucoma surgery treatment.
Setting:
Foreword prospective study of 50 eyes from 46 patients suffering from the open-angle glaucoma qualified for phacoemulsification combined with Ex Press device implantation (group I, n=27) and Ex-Press implantation alone (group II, n=23). The participants were 21 men and 25 women in age between 56 and 90 (77 average) hospitalized in Ophthalmology Clinic of the Medical University of Bialystok, Poland, in 2013-2014.
Methods:
The main diagnostic tests included measuring refraction, aplanation tonometry, keratometry, pachymetry, gonioscopy, ophthalmoscopy. Some perioperative and postoperative complications and intervations (bleb needling, 5-FU injections) were analyzed as well. Intraocular measurements was performed before the surgery and then on 7th, 14th, and 28th day after operation and 3,6,9 months and one year after the operation. Statistical methods like Mann- Whitney test and Wilcoxon test were used in this research.
Results:
In all patients a statistically significant decrease of intraocular pressure after surgery was observed (p <0.05). In the eyes, which underwent combined phaco- Ex-Press surgery (group I) - after 7 days the postop. IOP drop was 38,11 % (SD 23,61), after 6 months 34,69% (SD 19,6). After plain Ex-Press device implantation (group II) the postoperative decrease of intraocular pressure was 45,14 % (SD 18,01) at day 7 th and 27,81% (SD 20,08) after 6 months. The studies are preliminary, they are still continued, and the results will be verified of greater number of clinical material.
Conclusions:
Plain Ex-Press device implantation surgery causes less expressed reduction in intraocular pressure compared to combined phaco-Ex-Press surgery in 6 months follow up. It is also characterized with more safety profile and less number of interventions in the postoperative period FINANCIAL INTEREST: NONE
