Management of glaucoma drainage device exposure with allografts

Purpose:

To report the outcomes of Glaucoma Drainage Device (GDD) exposure repair.

Setting:

Ege University Faculty of Medicine, Department of Ophthalmology.

Methods:

The outcomes of 11 patients (7 male, 4 female) who underwent surgical repair for GDD exposure between 2011 and 2014 were analysed. Exposed GDDs were repaired with fascia lata allograft combined with conjunctival flap in 7 (63.6%) patients, and solely with conjunctival flap in 4 (36.4%) patients.

Results:

Mean age of the patients was 57.1±12.8 (range; 32 and 73) years. The primary indications for GDD implantation were silicone oil related glaucoma (5 patients), Sturge Weber Syndrome (1 patient), uveitis related glaucoma (2 patient), and bullous keratopathy associated glaucoma (3 patient). The mean time to exposure from the GDD procedure was 28.2±15.2 (range; 8 and 60) months. The exposure was over the tube in 4, and over the plate in 7 cases. Surgical success was achieved in all patients who underwent fascia lata allograft combined with conjunctival flap with a mean follow-up of 18.9±19.4 (range; 3 and 60) months. Additional repairing procedure with fascia lata was required in the 3 (75%) of 4 patients that were treated solely with conjunctival flap.

Conclusions:

Fascia lata allograft combined with conjunctival flap is an effective and permanent surgical procedure in cases with GDD exposure. Conjunctival flap reparing alone seems to be insufficient in these patients. FINANCIAL INTEREST: NONE
