The effect of a new microinvasive glaucoma surgery (MIGS) device on endothelial cell count: a one year double blind prospective randomized clinical study

Purpose:

One of the major complications of glaucoma tube surgery is the endothelial cell loss. Recently trabecular micro-shunts have been proved effective in reducing intraocular pressure in conjunction with cataract surgery, but there are no reports on their potential side effects on the endothelium

Setting:

Methods:

Patients with an endothelial cell density lower than 1500 or with endothelial guttae were excluded. Endothelial cell evaluation was performed with a Konan Cellchek XL. The percentual change of the preoperative and postopoerative CD, SD, CV and SA was compared between the two groups using unpaired Student t-test. All surgery was performed by the same surgeon (AF)

Results:

A total of 36 patients were included in the study using a 1 :1 randomization criterion (18 cases: 18 controls). Two patients in each group were excluded because their basal endothelial cell count was lower than 1500. Two POAG patients in the control group and one in the combined surgery group were lost to follow-up. Cases and controls had comparable pre-operative endothelial parameters (CD: controls: 2390.3+/-314 cases: 2435.4+/-275; SD: controls: 153.4+/-44 cases: 153.8+/-33; CV: controls: 35.3+/-8.1 cases: 36.6+/-7.3 SA: controls: 53+/-8.9 cases:57+/-9.4). The percentual decrease in cell density between groups (cases: 11,9%+/- 10.7; controls: 18.1%+/-13.3) after surgery was not significantly different (p=0.5).

Conclusions:

The Percentual CD reduction in both groups is comparable to the data reported in the literature. The implant of the Hydrus device in conjunction of cataract surgery does not seem to significantly affect the endothelial cell count up to one year. Longer term studies are warranted.
