Selective laser trabeculoplasty for glaucoma management after penetrating keratoplasty: when the patient has allergy to topical glaucoma treatment and refuses surgical intervention

Abstract Details

Purpose:

Intra-ocular pressure (IOP) elevation is a known post-Penetrating Keratoplasty (PKP) complication; usually treated by medications or surgical intervention. Selective Laser Trabeculoplasty (SLT) could be another option when traditional treatment is not applicable.

Setting:

King Khaled Eye Specialist Hospital, Saudi Arabia

Methods:

A 42 years old lady, suffering from Keratoconus and had PKP for her Right eye, 2 months post-PKP she developed IOP elevation, IOP was controlled by medications for 4 months, then she started to develop Periorbital contact dermatitis and severe allergic conjunctivitis, antiglaucoma medications were changed to preservative free medication , without improvement. Eventually most topical treatment were stopped, patient underwent SLT for inferior 180°, and 4 months later for superior 180°.

Results:

Patient had a starting IOP of 40mmHg, was well controlled on 3 topical antiglaucoma medications, following 180° SLT; IOP went down to 23mmHg and she was on 2 antiglaucoma medication, which were tapered to one medication & the second 180° SLT session was given, following which IOP became 19mmHg. On her last follow up visit; 17 months later, IOP was 14mmHg and she was on 1 antiglaucoma treatment, eye was quiet, with clear corneal graft and no change in visual acuity or cup/disc ratio.

Conclusions:

SLT is safe and effective alternative for the treatment of post-PKP IOP elevation. Patient might not show favorable response until 360° of the angle was treated. FINANCIAL INTEREST: NONE
