The outcomes of Ahmed glaucoma valve implantation in patients with glaucoma secondary uveitis

Abstract Details
Purpose:
To evaluate the clinical results of Ahmed Glaucoma Valve (AGV) Flexible Plate implantation in patients with refractory glaucoma secondary to uveitis who do not respond intensive medical therapy and / or conventional filtration surgery.
Setting:
Istanbul Education and Research Hospital Ophthalmology Department
Methods:
18 eyes of 16 patients who underwent AGV implantation in the Ophthalmology Department of Istanbul Education and Research Hospital were retrospectively analyzed. Male/Female ratio was 10/6, mean age was 32±14.1. Despite intensive medical treatment, intraocular pressure (IOP) could not be controlled before the AGV (model FP7) implantation. Visual acuity, IOP and the number of antiglaucomatous medications that we recorded pre-operatively were compared with those in the post-operative period. The surgery was deemed successful if the number of antiglaucomatous medications decreased and postoperative IOP levels were kept low without loss of light perception and visual complications.
Results:
The mean follow-up period was 19.2±3.4 months. The mean pre-operative IOP value was 32.27±8.41 mmHg. The mean post-operative IOP value was 16.73±7.59 mmHg. The difference between these levels was statistically significant (p?0.05). The mean number of antiglaucomatous medications was 2.80±0.77 pre-operatively and 0.54±0.62 post-operatively. Pre-operative visual acuity remained the same after surgery. In the first post-operative day, there was shallow anterior chamber and hypotony in 2 eyes (11.1%). Anterior chamber was created with viscoelastic agents in these cases. Minimal hyphema was observed in 2 (11.1%) eyes which was resolved spontaneously. Fibrin membrane was seen in 3 eyes (16.7%) which was resorbed with intensive topical corticosteroid treatment. Choroidal detachment developed in 1 eye (5.6%).
Conclusions:
The implantation of AGV (model FP7) is an effective surgical procedure in glaucoma secondary to uveitis because it reduces IOP levels and causes minimal complications. FINANCIAL INTEREST: NONE
