First clinical experience with femtosecond laser-assisted cataract surgery

Purpose:

To present personal clinical experience with FS laser-assisted cataract surgery utilizing VICTUS platform (B+L/VALEANT).

Setting:

S. Fyodorov Eye Microsurgery Complex State Institution, Moscow, Russia

Methods:

83 patients (83 eyes) with cataracts of different grades including 6 eyes with moderate zonular weakness and natural lens instability due to PEX syndrome were operated. All cases underwent laser capsulotomy and lens fragmentation. In 76 eyes clear corneal incisions were performed with disposable metal blade, in 7 eyes - with the laser.

Results:

No major complications were observed during or after the surgery. Nevertheless some specifics in combining laser and surgical steps of the procedure were mentioned. Mean duration of laser step was 7.4+/- 2.34 min (range 5-22 min). Interval in between the laser and surgical step was 38,9 +/- 20.8 min (from 7 to 86). Performing laser and surgical steps consecutively or in a short series (2 to 5) caused the above mentioned variability and increased time interval between the procedure steps. Pause in between the steps was leading to pupil constriction. The latter required changes in mydriatics instillation protocol. Anterior capsulotomy flap was «free floating» in 94,6%. At the beginning of the learning curve hydrodissection step was limited by the presence of the intralenticular air bubble. The most popular laser nucleus fragmentation patterns included 6 to 8 radial incisions, and combined radial-circular pattern. Bi-manual irrigation-aspiration followed by IOL implantation was uneventful. Preoperative BCVA (Snellen) 0.15 +/- .0.13 (range 0.001 - 0.7) improved to 0.74 +/- 0.25 (range 0.4 - 1.0). Mean follow-up period was 3 months.

Conclusions:

Femtosecond laser-assisted cataract surgery with VICTUS provided very good and consistent surgical outcomes. Learning curve is relatively flat, but requires some changes in surgical algorithms. FINANCIAL INTEREST: NONE
