Effects of a femtosecond laser method of phacofragmentation on effective phacoemulsification time in cataract surgery and refractive lens exchange

Purpose:

To assess the effective phacoemulsification time (EPT), the learning curve and comparative outcomes of a femtosecond laser method of anterior capsulotomy and laser phacofragmentation

Setting:

Optegra Eye Sciences

Methods:

Observational retrospective dual surgeon and dual location case series of 127 eyes. The operative parameters, intra and postoperative complications rates were examined and are reported for treatments of all grades of cataract with the next-generation laser (LensAR Laser System) and 3-D imaging technology for cataract & refractive surgery. Pre-operative LOCS III grading was used throughout.

Results:

Mean (SD) dock time was 131.9±18.2 seconds and phaco time was 26.1±14.0 seconds. Median phaco power was 14.1% and mean EPT was 3.87 ± 2.73 seconds. There were zero intraoperative complications. The only postoperative complications encountered were post-operative uveitis (2 cases, 1.6%) and raised IOP > 21mmHg (2 cases, 1.6%), reduced compared to internationally published benchmark incidence figures for non-laser treatment methods.

Conclusions:

The LensAR may allow for reduced trauma to the eye, and after a short learning curve preserves the quality of results while delivering advantages for the patient journey compared with non laser-assisted cataract surgery. Ultrasound energy applied and effective phacoemulsification time, as well as the incidence of intra- and post-operative complications, are reduced in this study compared to standard reference values used in conventional cataract & refractive surgery.
