Cross-linking and lens exchange to correct refraction problems on account of keratoconus

Abstract Details
Purpose:
we represent our experience of treatment and correction of refraction problems on account of keratokonic cornea.
Setting:
Ophthalmic clinic 'OKOMED', Moscow, Russia
Methods:
we investigate in research 16 patients (21 eyes) with keratokonus of 2-3 degrees (middle age of 28,5 years, range 21-37 years). All patients were investigated with expanded ophthalmology examination (including corneal topography, pahymetry, endothelial microscopy, OCT of a cornea). UF-crosslinking by a standard technique was performed by the first step. The second stage consisted lens exchange with implantation of toric monofocal IOL (AcrySof IQ toric) after stabilization the keratometric dates of a cornea (average 3-4 months). Clinical outcomes were evaluated in terms of predictability, safety, refractive and functional results (UDVA, CDVA). The eyes were followed-up from six months to one year after treatment.
Results:
Pre-op UDVA was 0,2±0,15 and CDVA 0,5±0,2. Cylindrical component of refraction was -4,0±1,75 D. After crosslinking optical results and refraction didn't change. Slightly decreased cornea thickness, in limits by 15±23 microns. After lens exchange and implantation of toric IOL UDVA was 0,6± 0,15 and CDVA was 0,8±0,1. Cylindrical component of refraction was -1,0±0,75 D.
We didn't observe statistically significant loss of a corneal endothelium. Subjective quality of vision assessment was very positive.
Conclusions:
Step-by-step treatment of keratoconus and the related refraction problems allows to reach optimal functional and optical results. The crosslinking is carried out for keratoconus stabilization. The solution of refraction problems is reached by carrying out lens exchange with toric IOL implantation. FINANCIAL INTEREST: NONE
