Сlinical outcomes of transepithelial corneal collagen cross-linking in treatment of corneal ectasia

Purpose:

Evaluate the clinical outcomes of transepithelial method of corneal crosslinking for patients with keratoectasia

Setting:

Ufa eye research institute. Clinical study of keratoectasia treatment using advansed technique of cross-linking.

Methods:

The material based on treatment of 54 patients (69 eyes) with primary ectasia . Among them: men - 44 (81.4 %), women - 25 (36.2% ), aged 22 to 60 years (mean age 37,5 ± 4,5). Mean follow-up of 2 years. Saturation of corneal photosensitizer - riboflavin 0.1 % sodium and 20 % dextran solution ('Dextralink ') was performed by the modified method of transepithelial using iontophoresis . Following parameters were used iontophoresis: an initial current of 0.2 mA with a gradual increase to 1.0 mA. Total saturation time of the corneal stroma - 10 minutes. Saturation efficiency showed in corneal biomicroscopy using cobalt color filter in the form of a yellow glow of the corneal stroma and the aqueous humor

Results:

In patients with ectasia 2 years after corneal collagen crosslinking transepithelial uncorrected visual acuity (UCVA) increased from 0,3 ± 0,08 to 0,43 ± 0,08; corrected visual acuity (CVA) increased from 0,56 ± 0 , 07 to 0,74 ± 0,09; keratometry data decreased from 47,16 ± 0,72 to 46,35 ± 0,68 D, summary corneal astigmatism decreased by 0,43 D; radius of corneal curvature increased by 0.13 mm, thickness of the cornea has decreased by an average of 27.0 microns. According to the maximum depth of the impact on OCT, transepithelial cross-linking of the cornea was determined by an average depth of 150-200 microns. In studying the aberrations of the optical system was determined by a qualitative assessment of the effectiveness of treatment. The second year of observation decreased major aberrations: total from 4,56 ± 0,55 to 3,4 ± 0,5, lower order aberrations (T Tilt) 2,79 ± 0,44 to 1 , 92 ± 0,16, higher-order aberrations with High 1,49 ± 0,17 to 1,21 ± 0,18, T. Coma - with 0,98 ± 0 , 13 to 0,61 ± 0,05, T. Trefoil - from 1,03 ± 0,13 to 0,96 ± 0,18

Conclusions:

Corneal collagen cross-linking using transepithelial iontophoresis clinically showed satisfing anatomic and functional outcomes FINANCIAL INTEREST: NONE
