Complication after cross-linking: case report

Abstract Details

Purpose:

In this case study we want to present Acanthamoeba keartitis as complication after cross linking.

Setting:

Specialty Eye Hospital 'Svjetlost', Zagreb, Croatia Medical School, University of Rijeka, Rijeka, Croatia

Methods:

43 years old male has keratoconus on both eyes. Cross linking (Dresden Protocol) was performed on his right eye and terapeutic contact lens was placed. After three days patient came with decreased visual acuity, pain and redness of the right eye. Anamnestically was found out that he used tap water to rinse his face. Conjucival swabs and surgical debridment of the corneal lesion were performed. Antibacterial, antifungal and antiamoebic eye drops and systemic antifungal therapy was given immediately.

Results:

Acanthamoeba was found in corneal scrapings. Patient was treated only conseravtively. After control corneal scrapings came negative, corticosteroids were incleuded in therapy. Patient reached preoperatively BCVA after 3 months and the cornea was almost completely clear. One year after cross linking, ICL toric - IOL was implanted in his right eye.

Conclusions:

Cross-linking is a low-invasive procedure with low complication and failure rate but it may have complications related to therapeutic soft contact lens, patient's discipline and cooporation during postoperative recovery. FINANCIAL INTEREST: NONE
