Purpose:
Evaluation of stability and functional response after riboflavin-UVA - induced cross-linking in patients with progressive keratoconus after 3 months follow-up.
Setting:
Yuzuncu Yil University
Methods:
Prospective nonrandomized trial conducted at the Department of Ophthalmology, Van Yuzuncu Yil University, Turkey. This study involved 58 patients with clinical and instrumental evidence of keratoconus progression aged between 13 and 33 years. We recorded ocular parameters of 66 eyes 58 patients with keratoconus in whom the riboflavin-UVA - induced corneal cross-linking was performed in all patients according to the standard epi-off protocol. The patients was divided into 2 groups according to corneal thickness (≥450 and <450 μm) at the time of enrollment. Parameters recorded preoperatively and postoperatively were as follows: VA(uncorrected visual acuity), BCVA (best spectacle - corrected visual acuity), autorefractometric readings, Orb-scan corneal topography, ultrasonic pachymetry, HRT III confocal microscopy (Rostock CorneaModule, Heidelberg, Germany) results.
Results:
Functional data of the patients showed an increase of 0.14 and 0.07 Snellen lines in VA and BCVA in the thicker group (corneal thickness ≥450) respectively and 0.18, 0.11 Snellen lines in the thinner group (corneal thickness <450 μm) at 3 months examinations. Patients in the thinner group already showed a better and faster functional recovery than the thicker group at 3-month follow-up. Topographic results showed improvement in K readings, but these were not statistically significant. Corneal thickness decreased statistically significant in both group (p=0,001) after the treatments. Improvement of spherical refraction was statistically significant in both group (p=0,001).
Conclusions:
The study demonstrated significant and rapid functional improvement in patients with progressive keratoconus, undergoing riboflavin-UVA - induced cross-linking. A good functional response and keratoconus stability was obtained after corneal cross-linking in a 3-month follow-up. FINANCIAL INTEREST: NONE
