Higher-order aberrations of the anterior and posterior corneal surfaces in eyes after keratoplasty procedures

Purpose:
To investigate higher-order aberrations (HOAs) of the anterior and posterior corneal surfaces after deep anterior keratoplasty (DALK), descemet stripping automated endothelial keratoplasty (DSAEK) and penetrating keratoplasty (PK).

Centro Hospitalar Lisboa Central
Methods:
A prospective study was conducted in 40 eyes (10 normal eyes, 10 eyes after DALK, 10 eyes after DSAEK and 10 eyes after PK). Corneal topography of anterior and posterior surfaces was obtained with a Scheimpflug-based corneal topographer (Pentacam®). Corneal HOAs in the center 6 mm diameter were calculated from the height data and were expanded with normalized Zernike polynomials and evaluated as root mean square values. Aberrometric parameters were correlated to visual acuity.
Results:
The mean anterior corneal surface total HOAs in the control group (0,12 +/- 0,03 μm) was inferior compared to DALK (1,36 +/- 0,14 μm), DSAEK (0,7 +/- 0,58 μm) and PK (2,16 +/- 1,48 μm) groups (p<0,01). The mean posterior corneal surface total HOAs in the control (0,06 +/- 0,01 μm) group was inferior to DALK (0,52 +/- 0,29 μm), DSAEK (0,24 +/- 0,11 μm) and PK (2,87 +/- 7,63 μm) groups (p<0,01). HOAs were not significantly correlated to visual acuity (p>0,05).
Conclusions:
Corneal HOAs are increased in keratoplasty procedures, particularly in DALK and PK. Our findings suggest that HOAs after keratoplasty may not play the central role in the visual outcome. FINANCIAL INTEREST: NONE
