Patients with Fuchs and cataract undergoing DSAEK and phacoemulsification: staged versus combined procedures

Abstract Details
Purpose:
To compare the surgical outcomes of staged and combined Phacoemulsification and DSAEK in patients with Fuch's endothelial dystrophy and cataract.
Setting:
Corneoplastic Unit and Eye Bank, Queen Victoria Hospital, East Grinstead, UK.
Methods:
Retrospective study of patients who had phacoemulsification followed within 3 months by DSAEK (group 1) or combined phacoemulsification and DSAEK (group 2). Patients who had previous eye surgery or any other ocular comorbidities were excluded.
Results:
There were 38 eyes in group 1 and 30 in group 2. There were no significant differences in the demographics and corneal tissue characteristics of the two groups. The endothelial disc dislocation and rebubbling rate within 1 week in group 1 was 10.5% while in group 2 was 33% (p=0.03), while the endothelial cell density was 1467±430SD for group 1 and 1650±318 for group 2 (p=0.1). The mean 12 month logMAR visual acuity was 0.43±0.38SD for group 1 and 0.34±0.29SD for group 2 (p=0.08). The overall graft failure rate at 12 months was 7.9% for group 1 and 3.3% for group 2 (p=0.6).
Conclusions:
Although the combined procedure seems to be associated with a higher complication rate the final outcomes seem to be similar in both methods. FINANCIAL INTEREST: NONE
