Peripheral corneal thickness after penetrating keratoplasty as a prognostic factor of grafted cornea

Purpose:

To evaluate the correlation between prognosis of grafted cornea and peripheral corneal thickness after penetrating keratoplasty.

Setting:

Department of Ophthalmology and Visual Science, St. Mary's Hospital, The Catholic University of Korea College of Medicine, Seoul, Korea

Methods:

We performed retrospective chart review about 111 eyes of 111 pateints who underwent penetrating keratoplasty from Oct. 2010 to Dec. 2013. Peripheral corneal thickness at 4 mm away from the center at 4 directions (12o/c, 3 o/c, 6 o/c, 9 o/c) were measured by using scanning-slit pachymetry at postoperative 1 year and averaged. We classified 111 eye`s by three groups according to peripheral corneal thickness, and compared and analyzed prevalence of graft failure after penetrating keratoplasty at each groups

Results:

The 40 eye`s of 111 eye`s were classed as thin peripheral cornea thickness group (averaged peripheral corneal thickness≤ 700㎛), 38 eye`s were classed as moderate peripheral corneal thickness group(averaged peripheral corneal thickness, 700 to 800㎛), 33 eye`s was classed as thick peripheral corneal thickness group(averaged peripheral corneal thickness> 800㎛). Prevalence rate of graft failure was 17.5% (7 eye`s) at thin thickness group, 13.2% (5eye`s) at moderate thickness group, 15.2%(5 eye`s) at thick thickness group. 2 year survival rate was 83.5% at thin thickness group, 81.5% at moderate thickness group, 77.6% at thick thickness group and there was no statistically significant difference among three groups (P=0.897). And in ROC curve, peripheral corneal thickness was not a important factor to predict and diagnosis graft failure.

Conclusions:

There was no significant correlation between postoperative averaged peripheral corneal thickness and function of grafted cornea. But further research is needed into long-term postoperative complications related to thickened peripheral cornea FINANCIAL INTEREST: NONE
