Penetrating keratoplasty in combined ophthalmopathology

Purpose:

Investigation into follow-up optical, biological and refraction results of one-stage optical reconstruction operations on the basis of penetrating keratoplasty in combined pathological alterations of the visual organ. Analysis of the total duration of the treatment and terms of medical rehabilitation.

Setting:

St.-Petersburg brunch of Sv. Fyodorov 'Eye Microsurgery Clinic', St.-Petersburg, Russia.

Methods:

Follow-up optical, biological and refraction results of 332 one-stage optical reconstruction operations on the basis of penetrating keratoplasty in patients with combined pathological alterations of the visual organ, were studied, including alterations involving various pathological changes in the anterior segment, in the anterior, posterior, and vitreous chambers of phakitic, aphakitic and artephakitic eyes. The control consisted of a group of patients, comparable in terms of quantitative and qualitative composition, whose operation had been performed in several stages. In all the operations, a fresh cornea taken no later than within 14 hours after the donor's death, was used, the endothelium density of the kerato-transplant being no less than 2000 cells per 1 square millimeter.

Results:

The 2 to 10 years follow-up results in the groups were as follows. The vision acuity average level with correction was 0.360.03 in the main group, and 0.380.01 in the control one. The biological results: transparent settling - 81.3%; semitransparent one - 12.7%; hazy one - 5.9% (6.0%). The refraction results: emmetropia in the groups was 75.9%, respectively; myopia 11.1%; hypermetropia 4.8%; astigmatism over 3.0 dioptries 8.2%. Total duration of the treatment in the control group was 2.0-2.5-fold that in the main group, the difference being statistically significant (Р<0.05).

Conclusions:

Follow-up optical, biological and refraction results of one-stage optical reconstruction operations on the basis of penetrating keratoplasty in patients with combined pathological alterations of the visual organ suggest a real possibility of improving the efficacy of surgical treatment for this category of patients, and a 2.0-2.5-fold acceleration of their medical rehabilitation.
