Hypotensive surgery in patients with corneal pathologу and glaucoma

Abstract Details



Purpose:

To assess long-term biological and hydrodynamic results of single-stage combined operations (a modified version of penetrating keratoplasty and trabeculectomy) in patients with corneal scarring complicated by secondary open-angle glaucoma. To compare the data obtained with those for operations performed in two stages.

Setting:

Academician S.N. Fedorov St.Petersburg Branch of the IR&TC 'Eye microsurgery', St.Petersburg, Russia.

Methods:

57 main study group patients with corneal leukoma of types II - III complicated by open-angle subcompensated stage I - III glaucoma underwent single-stage operations developed by the authors of the present study. 30 control group patients underwent trabeculectomy at stage 1, and penetrating keratoplasty at stage 2 with fresh 8 mm keratotransplants (endothelium density of no less than 2000 cells per square millimeter). The results were assessed 6-12-36 months after surgery.

Results:

: In the main group transparent engraftment was observed 6 months later in 84% of patients, 12 months later in 75%, 36 months later in 65%. In the control group transparent engraftment 6 months later constituted 86%, a year later 80%, 36 months later 67%. In the main group six months after the operation normal ocular pressure was observed in 88% of cases, in the control group ocular pressure normalized in 90% of cases. Three years later the hypotensive effect in the absence of medication was found in 83% of patients in the main group and 86% in the control group. The biological and hypotensive results of single-stage operations 6-12-36 months later were also significantly better than in the control group (p<0.05).

Conclusions:

The biological and hypotensive results of single-stage combined operations are comparable in the long-term with those obtained in the case of two-stage surgery. FINANCIAL INTEREST: NONE
