Clinical outcomes of Descemet's stripping automatic endothelial keratoplasty after graft creation with a keratotome versus a femtosecond laser

Purpose:

Clinical outcomes after surgery were compared after a keratotome or a femtosecond laser (intraLase) was used to create the graft in Descemet's stripping automatic endothelial keratoplasty (DSAEK)

Setting:

This study was performed at Dokkyo Medical University Hospital in Tochigi Prefecture, Japan.

Methods:

Forty-two eyes that underwent DSAEK between February 2008 and October 2010 were studied. DSAEK was performed with a keratotome (KT group) for 21 eyes and with a femtosecond laser (FS group) for 21 eyes. Corrected eyesight, endothelial cell density (ECD), and percent decrease in ECD were compared.

Results:

Corrected eyesight values before surgery and at 1 month, 3 months, and 6 months after surgery (LogMAR) were not significant different between the two groups, at 1.33±0.53, 0.57±0.28, 052±0.29, and 0.49±0.30 in the KT group and 1.17±0.43, 0.88±0.39, 0.55±0.27, and 0.46±0.23 in the FS group, respectively. ECD (cells/mm2) and percent decrease in ECD after surgery were also not significantly different between the two groups except that the FS group showed a greater percent decrease than the KT group at 6 months: in the KT group, ECD was 1614±471 at 1 month, 1514±441 at 3 months, and 1572±494 at 6 months and the percent decrease was 42%, 46%, and 44%, respectively; in the FS group, ECD was 1376±475, 1336±464, and 1179±331 and percent decrease was 51%, 55%, and 59%, respectively.

Conclusions:

Eyesight improved regardless of whether KT or FS was used to perform DSAEK. The decrease in ECD was greater when FS was used. FINANCIAL INTEREST: NONE
