Comparison of the visual and refractive outcomes of one versus two-segment femtolaser-assisted intrastromal corneal ring implantation in keratoconic subjects

Abstract Details



Purpose:

To comparison the efficacy and safety of one segment versus two-segment femtolaser assisted intrastromal corneal ring segment (ICRS) Intacs SK implantation in the treatment of keratoconus.

Setting:

Eye research center Rassoul Akram hospital, Iran University of medical sciences, Tehran- IRAN.

Methods:

In this prospective comparative study, 1 or 2 rings of Intacs SK ICRS were implanted using femtosecond laser in eyes with Stage I- IV KCN. Visual, refractive, corneal tomography changes and complications were analyzed over a 6-month follow-up. Postoperative visits were at 1 week, 2 and 6 months.

Results:

The study evaluated 71 eyes, 36 eyes of 26 patients received 1 segment (1S group) and 35 eyes of 29 patients had 2 segment (2S group). At 6 months, the spherical equivalent decreased by a mean of 1.15 and 2.98 D, the mean sphere decreased .63 and 2.37 D and the mean cylinder decreased 1.03 and 1.25 D in 1SG and 2SG respectively. The mean Pre op LogMAR UCVA was .75 that increased to .37 and .98 to .54 in 1SG and 2SG respectively. The mean Pre op LogMAR corrected distance visual acuity (CDVA) was .48 that increased to .19 and .62 to .36. Keratometry (K) flat and K steep decreased by a mean of 1.08 and 3.00 D and 2.15 and 3.83 D in 1SG and 2SG respectively (p=0.00). The mean defocus decreased by 1.67 and 3.60 D in 1SG and 2SG respectively (p=0.00). The mean corneal power in 3mm zone, the mean anterior BFS and the mean anterior chamber depth decreased significantly in both groups. 94.% and 91% of eyes gained 1-7 line of DCVA in 1SG and 2SG respectively. The mean posterior BFS and irregularity in 3mm and 5mm zone was not changed significantly.

Conclusions:

Femtolaser assisted implantation of one or two segment Intacs SK ICRS for keratoconus was safe and effective, leading to significant improvement in UDVA, CDVA, and refractive error. FINANCIAL INTEREST: NONE
