Abstract Details

Purpose:

To evaluate causes of recurrent keratoplasty and assess the effect prognostic factors on health of graft.

Setting:

Elvin Halili¹,Tahir Kansu Bozkurt²,Banu Acar¹,Suphi Acar³ Haydarpaşa Numune Training and Research Hospital,Eye Clinic,İstanbul¹ Ümraniye Training and Research Hospital,Eye Clinic ,İstanbul ² Çamlıca Universal Hospital ,Eye Clinic,İstanbul³

Methods:

The mean age was 57,4+ 18 years with 42 eyes of 41 patients with recurrent corneal transplant were enrolled in the study. Preoperatively, a complete ophthalmologic examination was performed to patient.Evaluating regreft health parameters and most recently applied after transplants were included in the study. No patient required additional surgical prosedures are not implemented.Postoperative evaluation was based on graft transparency which is an indicator of the grafts health.Graft vascularization or persistent corneal scarring regardless no healing edema was considered graft failure.Recurrent corneal transplants for patients who were followed for an average 62.6 months.

Results:

34 patients second times , 7 patients thırd tımes had undergone corneal transplantant. Our cases most common primary indication for keratoplasty is pseudobullosis keratopathy.Pseudobullosis keratopathy is the most common cause of complicated cataract surgery in our cases. The most common cause of corneal transplant is endothelial failure. The graft rejection cases between no rejection cases significant difference wasn't detected in terms of follow up(p=0,24). Analysis with Kaplan meier evaluated graft survi with operations due to keratitis no rejection, the maximum rejection pseudobullosis keratopathy, aphakik bullosis keratopathy, perforation, desmatosel cases, keratoconus , pellucid marginal degeneration , fuchs, dystrophy cases were identified at least rejection .The cases of 70%seen at least rejection transparent cornea is monitored in the 25th month. 60%per pseudobullosis keratopathy, aphakikbullosis keratopathy in 50th month, 60% of the cases of perforation, desmatosel seen rejection in 30 th month. The first graft survi between second graft no rejection significant relationship was detected with cox analysis(p=0,012).

Conclusions:

The most common cause of recurrent corneal transplant is endothelial failure. The maximum rejection seen pseudobulosis keratopathy, perforation groups, the minimum rejection seen keratoconus,dıstrophy groups. The first graft survi between the second graft health significant relationship have been identified. Graft life is linked with the primary indication. FINANCIAL INTEREST: NONE
