Topographic changes after suture removal in keratoconus patients who underwent penetrating keratoplasty

Abstract Details
Purpose:
To investigate topographic changes after suture removal in keratoconus patients underwent penetrating keratoplasty.

Methods:
This prospective study included 19 keratoconus patients underwent penetrating keratoplasty.Donor corneas were cut from the endothelial side with a disposable donor punch and sutured into the recipient bed with a 16-20 bite, single 10-0 nylon running suture. Sutures were removed after 1 year from surgery. Corneal topographic analysis was performed with Keratograph (Oculus, Wetzlar, Germany) just before and one month after suture removal. Flattest and steepest K values, magnitude and axis of the astigmatism, eccentricity, Q value, analyzed area and topographic patterns were recorded.
Results:
12 patients were male and 7 patients were female. Mean age was 38± 12.41 (18-66). Mean K increased from 41.94 ± 5.64 (31.05-49.20) to 45.44 ± 3.27 (41.20-55.20) D (p=0.029). Mean astigmatism increased from 4.53 to 5.77 D (p=0.13). Axis changed from 81.33 to 70.30 degree (p=0.47). Ecc value decreased from 0.50 to 0.13 (p=0.086). Mean Q value decreased from 11.79 to 11.71(p=0.43) Analyzed area increased from 37.68 to 44.89 % (p= 0.131). Prolate topographic patterns decreased from 36.84 % to 26.31 %, oblate patterns increased from 21.05 % to 42.10 % after suture removal.
Conclusions:
An 3.5 increase in mean K value after suture removal following PK means a myopic shift in refraction. 1.23 D increase in astigmatism after suture removal may be expected. Topographic analysis can be acquired in a wider area. Overall oblate and ellipsoid geometry of the cornea, judged by the Q and eccentricity values, remains the same after suture removal.
