Five-year graft survival comparing Descemet's stripping automated endothelial keratoplasty with penetrating keratoplasty in an Asian cohort

Abstract Details



Purpose:

To compare 5-year graft survival following Descemet's stripping automated endothelial keratoplasty (DSAEK) and conventional penetrating keratoplasty (PK) in Asian eyes.

Setting:

Tertiary Center

Methods:

We reviewed consecutive patients who underwent DSAEK (60 eyes) or PK (128 eyes) with Fuchs' endothelial dystrophy (FED) or bullous keratopathy (BK) from 2006 - 2008, with a minimum of 5-year clinical follow-up. Clinical data, donor and recipient characteristics were recorded from our prospective Singapore Corneal Transplant Study. All surgeries were performed by the 5 corneal surgeons at our centre, which included cases performed or partially performed by corneal fellows in training under direct supervision. Our main outcome measure was cumulative graft survival up to 5 years.

Results:

There were no significant differences in baseline characteristics of patients who underwent either DSAEK or PK. There was no difference in proportion of primary diagnosis (FED: 111 eyes, 59% versus BK: 77 eyes, 41%) between surgical techniques (P=0.855). Overall 5-year graft survival was comparable between DSAEK and PK (86.9% vs. 85.3%, log-rank P value = 0.199). Median 5-year endothelial cell loss was significantly greater in PK compared to DSAEK (71.7% vs. 46.9%, P<0.001). Graft survival was superior in DSAEK compared to PK in FED eyes (100% vs. 89.3%) but not in PBK eyes (82.7% vs. 80.0%, log-rank P value = 0.321).

Conclusions:

In Asian eyes from the same study cohort with a standard post-operative regime, 5-year graft survival was comparable between DSAEK and PK in eyes with FED and BK. FINANCIAL INTEREST: One of more of the authors... gains financially from product or procedure presented
