Femtosecond-assisted intrastromal corneal ring implantation for keratoconus: 1 year clinical and tomographic results

Purpose:

To evaluate clinical and tomographic parameters before and 1 year after femtosecond laser-assisted intrastromal corneal ring segment (ICRS) implantation.

Setting:

Instituto de Olhos Renato Ambrósio Rio de Janeiro, Brazil

Methods:

This retrospective study included 24 eyes of 18 patients (mean age, 19.2 years [range, 18.3 to 42.0 years]) with a diagnosis of keratoconus. Clinical assessments included measurements of uncorrected distance visual acuity (UDVA), manifest refraction, corrected distance visual acuity (CDVA), and corneal tomography (Pentacam, Oculus Gmb, Wetzlar, Germany) prior and one year after femtosecond laser-assisted implantation of the intrastromal corneal ring segment (fs-ICRS; Keraring, Mediphacos Ltda, Belo Horizonte, Brazil). Implantation was planned accordingly to Keraring Nomogram, including tomographic corneal elevation and thickness evaluations.

Results:

At the 1-year follow-up visit, there was statistically-significant improvement in both (LogMAR) UDVA (0.75 ±0.44 post-operatively versus 1.25 ±0.48 pre-operatively, p=0.0004) and (LogMAR) CDVA (0.20 ±0.14 post-operatively versus 0.39 ±0.25 pre-operatively, p<0.0001). There was also a statistically-significant improvement in mean spherical equivalent (-4.76 ± 4.23 post-operatively versus -6.34 ± -6.36 preoperatively, p=0.0093). Corneas were statistically les prolate in asphericity (Q front 30° less legative values) and there were also statistically significant improvements in K2, front and back elevation values at the thinnest point (Best-fit sphere for 8mm), keratometric astigmatism, and maximal keratometry (KMax) readings at the 1 year postoperative (p=<0.05).

Conclusions:

Implantation of the Keraring ICR effected a significant improvement in visual acuity and corneal flattening in this series of patients.
