Novel graft over host technique (Nawaf's) to counter positive posterior pressure during penetrating keratoplasty

Abstract Details

Purpose:

To describe a technique for safe penetrating keratoplasty (PK) in cases associated with positive posterior pressure (PPP).

Setting:

King Abdulla Medical City, Holy Makkah, KSA

Methods:

After 360 deg. partial trephination and 90 deg. full cutting of the host cornea, PPP occurred. This quadrant is secured by one suture. Then the adjacent quadrant is cut and secured again with a single suture till the host cornea is entirely separated but still remain sutured by 4 cardinal sutures. The graft is then sutured over the host cornea (after putting viscoelastic) with 4 sutures, each in between the previous 4 sutures. The host corneal button is then delivered out after cutting its sutures. Remaining graft is then precisely sutured with 12 more sutures.

Results:

Intraoperatively, the anterior chamber was maintained, and no complications of PPP occurred. postoperatively, the cornea was clear and the astigmatism and endothelial cell count were within usual ranges

Conclusions:

This technique is safe and effective to prevent complications of PPP during PK FINANCIAL INTEREST: NONE
