Bilateral corneal trophic ulcers and subsequent infectious keratitis in a patient with graft-versus-host disease

Abstract Details

Purpose:

To report the clinical picture and the therapeutic approach to a patient with chronic graft versus host disease (GVHD). He developed severe dry eye and bilateral trophic corneal ulcers subsequent bacterial keratitis with hypopyon formation.

Setting:

Bayindir Hospital, Departmentof Ophthalmology

Methods:

A 54-year-old men with chronic myeloid leukemia, who had had a bone marrow transplant subsequently developed chronic graft versus host disease (GVHD). He developed severe dry eye and bilateral trophic corneal ulcers.

Results:

The patient was treated with autologous serum, topical unpreserved topical steroids topical cyclosporine A, unpreserved artificial tears and therapeutic contact lenses as well as systemic immunosuppression. While wearing extended wear soft contact lenses, he developed in her left eye a bacterial ulcer with hypopyon that responded to topical fortified vancomycine and ceftazidime therapy. Conjunctival and corneal cultures were negative.

Conclusions:

Patients with chronic GVHD can develop severe inflammatory changes in the ocular surface, with the development of severe dry eye and sterile, as well as infectious, keratitis. GVHD patients should be carefully evaluated for such severe complications. FINANCIAL INTEREST: NONE
