IOL of choice in MICS and MIVS combined surgery

Abstract Details



Purpose:

To explain the reasons for our personal choice of four haptics IOLs in MICS and MIVS combined surgery (micro-incision cataract surgery and microincisional vitrectomy surgery), by observing the intraoperative and postoperative stability of IOL into the capsular bag after air or gas infusion into the vitreous cavity.

Setting:

Department of Ophthalmology San Bassiano Hospital, Bassano del Grappa (VI), Italy

Methods:

20 eyes affected by cataract and macular hole or rhegmatogenous retinal detachment underwent MICS and MIVS combined procedure with air or gas infusion in the posterior chamber. None of them had showed any pathologies or malformations in the anterior segment before surgery. Ten eyes were implanted with two haptics IOLs (group 1) and ten with four haptics IOLs (group 2). Intraoperative observation and postoperative one-week follow up were done to evaluate IOL stability into the capsular bag.

Results:

In three eyes of group 1 ocular hypertension with pain was observed in the first postoperative day caused by pupillary capture of the tilted IOL optic . They needed urgent surgical treatment in order to solve the postoperative pupillary block by repositioning the lens. No intraocular lens tilt and decentration due to air or gas pushing towards the capsular bag were observed in group 2.

Conclusions:

Suspended in the capsular bag between four points instead of two, four haptics IOLs showed a better stability compared to two haptics IOLs when posterior chamber is filled with air or gas. Four haptics IOLs are intraocular lenses of our choice in MICS and MIVS in order to avoid any anterior segment complications due to air or gas pushing from back such as IOL tilt and decentration with pupillary capture and subsequent pupillary block glaucoma.
