Combined cataract and iris tumor surgery: special considerations

Abstract Details



Purpose:

We report a case of simultaneous cataract and iris melanoma surgery and highlight possible pitfalls and usefull tips when performing such procedure.

Setting:

A 60 years old lady reffered to our clinic for cataract removal. Close examination revealed vascularized iris tumor, most likely a melanoma, and decision has been made to proceed with combined surgery.

Methods:

Under peribulbar anesthesia our patient underwent phacoemulsification, IOL implantation, creation of scleral flap, tumor resection and pupilloplasty.

Results:

On the first postoperative day the patient's best corrected visual acuity was 1.0. The patient was satisfied with functional and cosmetic result and had no glare symptoms. Histopathological analysis revealed borderline lesion with high malignant potential.

Conclusions:

Management decisions for iris tumors depend on clinical features but also on patient's wishes and demands. Our patient was very satisfied with results and would do the same procedure all over again. We should consider the role of prophylactic surgery for managing individuals with such conditions, especially when considering advances in microsurgical techniques over the past 20 years.
