Unusual presentation of an intraocular foreign body retained for sixty years

Abstract Details
Purpose:
To describe how to manage missed intraocular stone foreign body that had remained asymptomatic underneath the iris for 60 years and appeared during cataract surgery.
Setting:
Baskent University Hospital, Ophthalmology Department, Ankara/Turkey.
Methods:
A 74-year-old female presented with progressive blurring of vision in the left eye for 7 years. She had a history of ocular trauma when she was 14 years old and she had not received any medical treatment. On presentation in the left eye, best-corrected visual acuity was only light perception and intraocular pressure was 15 mmHg. Biomicroscopic examination revealed intumescent cataract, corneal scar at five o'clock position and anterior synechiae. Extracapsular cataract extraction was performed for the left eye under local anesthesia. During the operation, it was recognized that a 3 mm diameter sized piece of stone embedded underneath the iris at 3 o'clock position and it was removed easily by using forceps. Cataract operation was completed with replacing seconder intraocular lens. Postoperatively the patient received topical moxifloxacin (0.5%) and prednisolone acetate (0.125%) eye drops.
Results:
At one week and one month postoperatively, the cornea and anterior chamber were clear, the lens was confirmed to be properly placed and her visual acuity was 20/30 in the left eye.
Conclusions:
Suspected retained foreign bodies should be 94 considered and evaluated before surgeries for preventing complications in patients with a history of penetrating ocular trauma even if there is not any symptom or evidence. FINANCIAL INTEREST: NONE
