Toric lens implantation in the treatment of post-keratoplasty astigmatism

Abstract Details
Purpose:
To compare two different toric IOL models independent of their rotational behaviour and efficiency in the treatment of cataract and pre-existing corneal astigmatism after penetrating keratoplasty (PK).
Setting:
Department of Ophthalmology, University Medicine Charité Berlin, Berlin, Germany.
Methods:
In this retrospective cohort outcome study, nine-teen eyes from 19 patients with coexisting cataract and corneal astigmatism after PK were included. Group 1 included nine patients (SA60TT, Alcon Inc.), and group 2 included ten patients (AT Torbi 709M, Carl Zeiss Meditec). Best-corrected visual acuity (BCVA) and best-uncorrected visual acuity (UCVA) in logMAR, refractive cylinder, and mean IOL rotation were determined one day, one week, and three months postoperatively.
Results:
The UCVA in group 1 decreased from 0.93 ± 0.36 preoperatively to 0.41 ± 0.33 after three months (p=0.011), and in group 2 from 1.13 ± 0.33 to 0.14 ± 0.12 (p<0.001).
The BCVA in group 1 decreased from 0.64 ± 0.37 preoperatively to 0.19 ± 0.12 after three months (p=0.004), and in group 2 from 0.66 ± 0.46 to 0.1 ± 0.09 (p<0.001).
Refractive cylinder in group 1 decreased from 4.95 D ± 2.99 preoperatively to 1.41 D ± 1.45 after three months (p=0.001), and in group 2 from 4.83 D ± 2.45 to 1.00 D ± 0.43 (p<0.001). Group 1 showed little IOL rotation over time, whereas group 2 remained stable during the follow-up period.
Conclusions:
Both toric IOL models offered strong rotational stability and provided satisfactory visual recovery in patients with corneal astigmatism after PK. FINANCIAL INTEREST: NONE
