Cataract surgery in a patient with congenital aniridia and total corneal neovascularization

Abstract Details



Purpose:

To report the procedure and outcome of cataracy surgery in a patient with Congenital Aniridia and total corneal neovascularization.

Setting:

Baskent University Faculty of Medicine, Department of Ophthalmology, Ankara, Turkey.

Methods:

Interventional case report.

Results:

A 37 years old female patient with bilateral Congenital Aniridia admitted to our clinic with decreased vision in her right eye. She had been treated for bilateral limbal stem cell deficiency, total corneal neovascularization and recurrent corneal epitelial defects in our clinic for a period of 8 years. She had a history of limbal allograft transplantation and consequent cataract surgery 6 years ago on her left eye. Altough corneal neovascularization was stable in her right eye, she complained of decreased vision over the last 6 months. Ocular examination revealed total corneal neovascularization and lameller nuclear sclerosis in her right eye. Corrected visual acuity in the right eye had decreased from 40/100 to 5/100 during a follw-up period of 6 months. We decided to perform a cataract excision. Trypan blue and retroillumination technique were used to increase the visualization of the lens capsule during capsulotomy. Extracapsular cataract extraction and posterior capsule IOL implantation was performed without any complications. Postoperatively the intraocular lens was well centered and the visual outcome was good. Her final visual acuity on the postoperative 4th week was 40/100.

Conclusions:

Cataract extraction in Congenital Aniridia with corneal neovascularization is challenging due to poor visualization during surgery and a high risk of capsular and zonular weakness. Trypan blue and retroillumination may be helpfull to perform an uncomplicated surgery. FINANCIAL INTEREST: NONE
