Relationship between lens density using Scheimpflug system and phacodynamics according to the age in nuclear cataract

Purpose:

We previously reported a positive correlation between lens density using Scheimpflug system and phacodynamics in nuclear cataract. The purpose of this study is to evaluate the relationship between lens density using Scheimpflug system and phacodynamics according to the age in nuclear cataract.

Setting:

Department of Ophthalmology, Seoul St. Mary's Hospital, The Catholic University of Korea, Seoul, South Korea

Methods:

In 241 eyes with nuclear cataract, the objective lens density (0 to 100 points) was determined using the Scheimpflug system while subjective grading was achieved using the Lens Opacities Classification System III (LOCS III). The cumulated dissipated energy (CDE) was obtained from the phaco machine after surgery.

Results:

In overall patients, CDE had a positive correlation with Scheimpflug lens density (r2=0.554, p<0.001) and LOCS III nuclear scores (r2=0.511, p<0.001). When it was analyzed according to the age, CDE had a positive correlation with lens density in patients in the forties (r2=0.424, p<0.01), in the fifties (r2=0.423, p<0.01), in the sixties (r2=0.519, p<0.01), in the seventies (r2=0.620, p<0.0001), and in the eighties (r2=0.719, p<0.01). LOCS III nuclear scores had a positive correlation with CDE in in patients in the forties (r2=0.539, p<0.001), in the fifties (r2=0.505, p<0.001), in the sixties (r2=0.542, p<0.001), in the seventies (r2=0.554, p<0.001), and in the eighties (r2=0.609, p<0.05).

Conclusions:

Preoperatively Scheimpflug lens densitometry is more useful to predict phacodynamic than LOCS III scores in nuclear cataract patients especially in old age after 70. FINANCIAL INTEREST: NONE
